Requesting Keys

EMPLOYEES THAT NEED TO REQUEST A KEY/S

 

  1. Complete the request form (PDF form).
  2. Requester emails the request form to their department head while carbon copying their Vice President.
  3. Department head will create a work order using FMX.
    1. Attach request form within FMX work request.
    2. FMX will track and email the department head on every status change.
    3. FMX instructional video for submitting a work request.
  4. Facilities will assign the work order.
  5. Locksmith will:
    1. Create key/s
    2. Deliver key/s to the facilities office.
    3. Complete the work request.
    4. Email the key holder where to pick up their keys.
  6. Facilities will close the work order.
  7. Key holder will show their employee ID.
  8. Key holder will sign the key agreement and receive their key.
